Information for Consent
Dermanude Skin Clinic wishes to help you make informed decisions about your treatment options and any relevant alternative options. You may at any time decline treatment even after giving your consent.
Purpose of Treatment – You have presented with concerns which have formed the basis of a clinical discussion and examination. The purpose of the proposed treatment is to address your concerns either individually or in combination with other modalities of treatment.
Outcomes – Your practitioner will endeavour in good faith to employ the principles of best practice in delivering your treatment. Each patient is individual and response to treatment will vary from patient to patient and treatment to treatment. As such it is difficult to guarantee outcomes will always meet your expectations.
Code of conduct- Your practitioner will be working closely alongside you to help you to reach your skincare goals. We remind clients that they are to speak to their skin expert in a respectful manner, any abuse will not be tolerated. If you do not maintain a respectful relationship with your skin expert, Dermanude Skin Clinic reserves the right to end communication with no refund.
The main skincare products used at Dermanude Skin Clinic is Obagi Medical. Obagi products are designed to help minimise the appearance of premature skin ageing, skin damage, hyperpigmentation, acne, and sun damage. Each product in the Obagi range is created for a distinct purpose and the product(s) selected to address your concerns have been indicated in your medical record.
Commonly Experienced Adverse Events
Redness Dryness/Flakiness/Peeling Skin Sensitivity or itching, Temporary worsening of acne, Tight feeling of the skin.
Less Common Risks
Allergic reactions, Hyperpigmentation/ hypopigmentation, Infection
Safety Prole
Although there is a proven record of safety with Obagi products, some people may be sensitive to certain ingredients which will require patch testing prior to use. It may be possible to complete this patch test at home with approval from your consultant. Specific instructions on how to carry out the patch test are provided at the end of this document.
Contraindications and Relative Contraindications to Treatment
• If you are pregnant or breast feeding
• Recent natural or artificial exposure to UV light resulting in a burn
• Eczema
• Active skin infections
• Specific conditions as outlined by your clinician relevant to the products you will be using
• Allergies to any ingredients within products (a test patch may be required in certain circumstances prior to treatment)
Limited or no clinical data exists regarding the efficacy and tolerance of these products in patients having a history of, or currently suffering from, auto-immune disease or auto-immune deficiency or being under immunosuppressive therapy. Your clinician shall therefore decide on the indication on a case by case basis according to the nature of the disease and its treatment and the need for monitoring. Your clinician will discuss the need for a preliminary skin testing for hypersensitivity if necessary, or in the case of patients with severe or multiple allergies.
Your clinician will also discuss the suitability of the product(s) having considered your medical history and any medications you currently take, as appropriate. As such, it is imperative you disclose such medications during your consultation.
For patients receiving Prescription Vitamin A therapy* please be aware of the following information
Therapy with prescription Vitamin A may make your skin more susceptible to sunburn and other adverse effects of the sun, so unprotected exposure to natural or artificial sunlight should be minimized. When outside, even on hazy days, areas treated with Vitamin A should be protected. An effective sunscreen should be used any time you are outside (consult your clinician for a recommendation of an SPF level which will provide you with the necessary level of protection).
For extended sun exposure, protective clothing, like a hat, should be worn. Do not use artificial sunlamps while you are using prescription Vitamin A. If you do become sunburned, stop your therapy until your skin has recovered. Extremes of climate tend to dry or burn normal skin. Skin treated with prescription Vitamin A may be more vulnerable to these extremes. Your clinician can recommend ways to manage your acne treatment under such conditions. *Prescription Vitamin A is part of the standard protocol for the following Obagi system: Nu-Derm, and may be prescribed as part of other treatment plans as discussed with your clinician.
Rare Risks
The skin of certain sensitive individuals may become excessively red, swollen, blistered or crusted. If you are experiencing severe or persistent irritation, discontinue the use of the product and consult your physician. There have been reports that, in some patients, the treatment area may develop a temporary increase or decrease in the amount of skin pigment (colour) present. The pigment in these areas returned to normal either when the skin was allowed to adjust to prescription Vitamin A, or therapy was discontinued. Drug induced Steven-Johnsons syndrome and toxic epidermal necrosis (SJS/TEN) have been reported as a possible risk associated in patients receiving therapies in the following categories: anticonvulsants, antibiotics (and specifically tetracyclines), antifungals, NSADs, antimalarials, allopurinol, and corticosteroids. Although tretinoin has not been directly linked to SJS/TEN, it may increase your risk.
Additional Information
Do not use other medications with prescription Vitamin A which are not recommended by your clinician. Using other medications not approved by your clinician may increase the risk of unnecessary redness or peeling.

At times we may provide discount codes to use on the ordering portal for retail products, the discount code provided will be specific to you, if anyone else is seen to be using it, or if you are ordering products from the portal for others using the code provided, you will not be eligible for the discounts during the remainder of your skin coaching programme and will have to purchase full price products.
If you purchase products from anywhere else but the Dermanude Skin Clinic portal, we cannot authenticate these products, be responsible for their outcomes, or provide any further support, we therefore have the right to stop your virtual skin coaching programme.
What to expect with your new skincare plan
• Prescription Vitamin A works deep inside your skin and this takes time. You cannot make it work any faster by applying more than one dose each day, but an excess amount of Vitamin A may irritate your skin. Be patient.

• There may be some discomfort or peeling during the early days of treatment. Some patients also notice that their skin begins to take on a blush.
• These reactions do not happen to everyone. If they do, it is just your skin adjusting to Vitamin A and this usually subsides within two to four weeks.
• These reactions can usually be minimised by following instructions carefully. Should the effects become excessively troublesome, consult your doctor.
For patients receiving prescription depigmentation products* please be aware of the following information
Prescription depigmentation ingredients are used to reduce the pigmentation of the skin by inhibiting the production of melanin, they may be indicated in conditions such as chloasma, melasma, freckles, senile lentigines, or to treat other areas of unwanted melanin hyperpigmentation. Treatment with prescription ingredients is often well tolerated, however a patch test is recommended to confirm tolerance and reduce the likelihood of an allergic reaction. If no reaction occurs when the test patch is conducted, then your clinician will advise you to continue treatment. Treatment should be limited to a relatively small area of the body at one time, as some patients may experience transient skin reddening and a mild burning sensation, neither of which preclude treatment. After application, it is important to wash your hands thoroughly to prevent any unwanted lightening of the skin. Pigment reduction seen during treatment with these ingredients may be reversed on the discontinuation of therapy and subsequent exposure to sunlight or other sources of ultraviolet radiation. The use of sunscreen or a physical barrier covering the treated area is therefore recommended to reduce the chance of pigmentation recurring.

The overall prevalence of sulfite sensitivity in the general population is unknown, although likely low. Sulfite sensitivity is seen more frequently in asthmatic people than in non asthmatic people. Prolonged use of prescription depigmentation ingredients has an extremely rare risk of causing exogenous ochronosis, a blueblack pigmentation with caviar-like papules on the skin. Any patient undergoing treatment with these ingredients should therefore be monitored closely and if you believe you may be showing any abnormal pigmentation, discontinue treatment immediately and see your clinician. Patch Testing
Patients beginning therapy with prescription depigmentation ingredients may be asked to undergo patch testing to evaluate skin sensitivity. This can be carried out either in clinic, or in the patient’s home as directed. To conduct a patch test, apply a small amount of the product to unbroken skin and check the area in 24 hours. Minor redness is not a contraindication, but where there is itching or vesicle formation or excessive inflammatory response, further treatment is not advised. Please contact your clinician immediately if you believe you are having an adverse reaction at any point during the patch testing or during the course of your treatment. From time to time, your clinician may recommend patch testing for non-prescription items.

Our prescriber will have access to your intake form, medical information, and the pictures you have submitted. In order to be approved for prescription items it is essential that the prescriber has all the information relating to your skin concerns and your health overall. It is crucial that you answer all questions honestly so that we can make a fair and accurate medical assessment of your skin and its requirements.
Returns & Refunds
If for any reason you’re not completely satisfied with your service or product, please contact the clinic within 14 days. All opened or used products cannot be returned. All prescription items cannot be returned to Dermanude Skin Clinic due to them being prescribed for your individual condition.
If for any reason you believe you have had an adverse reaction to any skin products from Dermanude Skin Clinic, you must submit all evidence including photos, written documentation and complete a Dermanude product reaction form, to ensure we can open a formal investigation with the specific skincare company. Dermanude Skin Clinic reserves the right to only provide funds back to the patient if the providing company has approved the adverse event and reimbursed Dermanude Skin Clinic.
The Virtual Skin Programme delivers a bespoke skincare programme based on your skin concerns and skin type. You will be in communication with your skin expert via WhatsApp only, as this is the most effective way for both you and your skin expert to communicate.
You will receive contact from your skin expert within 48 hours of completion of this form, they may ask for more information regarding your concern in order to ensure the correct products are selected. You will be provided with your bespoke skincare plan within 48 hours of the skin expert receiving all the information they have requested (if no additional information is required you will receive your plan within 48 hours of completing this intake form).
