A standard peeling is a treatment that acts on the surface of the skin by chemically eliminating the top layers and contributing to its renewal. Chemical peels deliver high concentrations of exfoliating acids to the skin to brighten the complexion, even skin tone and to improve the appearance of spots and wrinkles. In the case of patients with dark spots, a peel can help fade the spots but the blemish will appear again if the mechanisms that produce it are not treated.
The cosmelan® method visibly removes and attenuates spots (just like a peel) but it most importantly provides a regulating action as it corrects the overproduction of pigmentation to prevent the spots’ reappearance. While peelings act on the surface of the problem, cosmelan® treatment targets the origin.

How does cosmelan® peel work?
Although many people refer to it as “cosmelan® peel”, the treatment consists of 4 phases that turn it into a protocol for the elimination and control of the spots rather than a one-time remedy.
Phase 1: intense depigmentation at home & in clinic month 1:
The treatment plan consists of an at-home prep phase lasting for 28 days. The protocol consists of:
– brightening foam cleanser: creamy cleansing foam with AHA’s.
– age element brightening concentrate: concentrated serum with brightening action and antioxidant active ingredients. Unifies the skin tone.
– melan 130 pigment control: very high solar protection and colour.
This is followed by your in-clinic appointment, our skincare professional applies cosmelan 1 mask for an intensive depigmenting action. We must let the mask act for 8 hours. This produces the removal of the melanin deposits of the skin, as well as its renewal.
Phases 2: depigmentation, regulation and control month 2:
In the first 7 days after your cosmelan treatment you will have a really basic routine, composing of the brightening foam & melan 130 spf (you have used these in your prep kit) alongside the melan recovery.
7 days after removing the Cosmelan One mask it is necessary to start your home protocol recommended by our skincare professional. The patient must follow an application guideline of cosmeceuticals complementary to the method. This is part of the depigmentation process and also the way to prevent the reappearance of spots. The protocol includes:
– brightening foam cleanser: creamy cleansing foam with AHA’s
– cosmelan 2: facial cream for removing and/or reducing spots of melanic origin.
– melan recovery: balm with calming and healing effect.
– melan 130 pigment control: very high solar protection and colour.
Instructions for use: apply a pea sized amount of Cosmelan® 2 twice per day (morning and night), followed by Melan Recovery and Melan SPF 130+ Pigment Control after daytime applications.
In the case of extreme sensitivity during this phase, consider spacing out the applications of Cosmelan® 2, without abandoning the treatment altogether. We will require you to watch our in clinic video to demonstrate application of products during consultation, and answer any questions you may have with the protocol.
Phases 3: depigmentation, regulation and control months 3-6
At this point, it is important to treat the root cause of the blemish by regulating overproduction of melanin through constant regulation of the melanocyte. This controls both intra- and intercellular melanin production and prevents re-pigmentation.
Instructions for use: apply Cosmelan® 2 morning and night, followed by Melan Recovery, and Melan SPF 130+ Pigment Control during the day. In case of sun exposure, reapply UV protection at midday.
The skin should start to look ‘normal’ at this stage and the hyperpigmentation should be vastly reduced.
Phases 4: depigmentation, regulation and control months 7
The tendency to develop dark spots represents an ongoing risk. During the control phase, the treatment prevents the reappearance of the treated spots and the formation of new ones, for a longer-lasting result.
Instructions for use: apply Cosmelan® 2 at night, followed by Melan Recovery. During the day, apply Melan SPF 130+ Pigment Control and reapply in cases of direct sun exposure.
The treatment is now complete. We strongly recommend continuous use of sun protection as a permanent cosmetic treatment.
What causes hyperpigmentation?
A common cause of hyperpigmentation is an excess production of melanin. Melanin is a pigment that gives skin its colour. It’s produced by skin cells called melanocytes. Several different conditions or factors can alter the production of melanin in your body.
- Certain medications can cause hyperpigmentation. Also, some chemotherapy drugs can cause hyperpigmentation as a side effect.
- Pregnancy changes hormone levels and can affect melanin production in some women.
- A rare endocrine disease called Addison’s disease can produce hyperpigmentation that’s most obvious in areas of sun exposure, such as the face, neck, and hands, and areas exposure to friction, such as elbows and knees.
- The hyperpigmentation is a direct result of an increased level of a hormone in your body that results in increased melanin synthesis.
- Excessive sun exposure can also cause an increase in melanin.
What is Cosmelan and who does it work for?
The Cosmelan® treatment is the world’s leading professional pigmentation removal method. The treatment is suitable for all forms of hyper-pigmentation from melasma, post-inflammatory hyper-pigmentation, solar lentigo to ephelides (freckles) and can be used on all skin phototypes, including any with dark or Asian skin tones.
How does Cosmelan work?
With an intensive corrective effect that regulates overproduction of melanin in the melanocytes, inhibiting and controlling the appearance of new dark spots. Its dual corrective and controlling action achieves both short and long-term results by keeping hyperpigmentation under control. The Cosmelan® contains an exclusive combination of active substances with proven efficacy, which act on all phases of the hyper-pigmentation process totally eliminating existing hyper-pigmentation, preventing its reappearance, and stopping the appearance of new dark spots.
What does the Cosmelan method involve?
First you will book an consultation through our online booking system. At consultation we will discuss your eligibility for treatment, medical history, carryout a skin analysis with out skin specialist using our skin scanner, and answer any of your questions.
The treatment consists of an at-home prep phase lasting for 28 days, this is followed by one in-clinic appointment and some home maintenance, which should be continued for ideally at least 6 months post peel.
After your consultation, you will be provided with your at home prep kit. This must be used for 4 weeks before the Cosmelan® in-clinic treatment and will prepare the skin for the treatment removing any dead skin cells, hydrating the skin, and allowing the Cosmelan® to work deeper within the skin. Your first in clinic treatment will be the application of the Cosmelan® One.
The Cosmelan® One is the professional treatment mask that is applied by our skin professionals in the clinic, and this should be left on at home for a period of 8 hours.
The Cosmelan® Two should then be used by you at home. This process is started 7 days post the in-clinic mask (Cosmelan® One) and is recommended to be used two times daily in the first month. It is VITAL that you follow our pre, during and post-procedure aftercare advice following your Cosmelan® peel. Not doing so can greatly affect your final results and may harm your skin.
Why is it important to use the homecare provided?
The preparation phase and home care process of Cosmelan® Two maintenance cream is continued to allow the melanocyte to stop producing the same abnormal production of melanin and continue to not produce this by its self. If this is not followed for the recommended amount of time the hyper-pigmentation will most likely return as the melanocyte will start producing the same hyper-pigmentation. Listening to our Skin Health Professionals at Dermanude Skin Clinic is vital. By over usage of Cosmelan Two® and not continuing or following our home care protocols, side effects can start to happen. We also cannot guarantee or cover any clients who have purchased Mesoestetic® homecare products from an unauthorised retailer i.e online sites or unlicensed clinics. We cannot guarantee the authenticity of these products so cannot continue to treat you.
How quickly will I see results?
Results can be seen in as little as two weeks but can continue to improve with the best of the result being seen between 30 and 60 days following the treatment. We recommend maintenance using the Mesoestetic® Depigmentation Home Care Pack and sessions of our Dermalux LED PhotoTherapy Pigmentation Protocol.
What is the downtime for this treatment and are there any side effects?
Peeling, a little redness, and irritation can be expected initially. Mesoestetic® Fast Skin Repair is recommended to be applied to the skin to reduce the peeling and irritation significantly, this is provided in the Cosmelan 7 Month System. The skin can become dry when reaching the stage of the programme when you start to introduce Cosmelan® 2 (this is completely normal). Some clients have also reported mild swelling in the first week.
Are there any aftercare products that should be avoided and is it safe to use other products at any point during the method?
Any products containing Vitamin C, Acids or Retinols should be avoided throughout the treatment. These will be too stimulating for the skin and could cause a reaction. Any other skin brands during this time especially cosmeceutical ranges should be heavily avoided.
During the first month, the skin will be particularly sensitised and we would therefore only recommend the use of the products contained in Cosmelan 6 month discolouration programme.
Can the treatment be combined with any other treatments?
During the Cosmelan® treatment, no other invasive skin procedures should be performed within the first 3 months. We highly recommend sessions of Dermalux LED Phototherapy sessions 3 months post peel to boost results and maintain your pigmentation-free appearance (you have 4 Dermalux sessions included free of charge in the Cosmelan 7 month discolouration programme). Once 6 months has passed from your in-clinic Cosmelan treatment, we can introduce other in clinic treatments such as SkinPen Microneedling, HydraFacial, AlumierMD & Chemical Peels.
Why should I choose Cosmelan over other treatments for hyperpigmentation such as lasers?
Lasers are commonly used to treat hyperpigmentation, whilst they can be a highly effective treatment for low and medium phototype skins (phototype 1-3) where hyperpigmentation is more visible on the skin surface. When using lasers to treat discolouration a maintenance treatment will always be required to prevent the reappearance of hyperpigmentation. In higher phototypes (phototype 4-6) however, the laser is less effective and the risk of post-inflammatory hyperpigmentation is much higher. The Cosmelan® method is better tolerated, more efficient, and involves a lower risk of recurrence with a longer-lasting result.
I am 6 months into my cosmelan method, why is my pigmentation returning?
It is most likely the pigmentation is not returning, when you have deeper pigmentation to start with this can take time to come to the surface. Dermal pigmentation can work its way up through the skins layers and start to be visible from 4 weeks to 9 months after the initial cosmelan treatment. This is normal and it is the only way it can be removed. Maintenance treatments can be performed using the Melanostop® Superficial Prep Resurfacing peel & Dermalux LED Light Pigmentation to encourage its removal.
